This application is exclusively for heart disease patients participating in the "Integrated Telemonitoring to Optimize Drug Titration for Acute Heart Failure with Reduced Left Ventricular Ejection Fraction: a feasibility study." This study is conducted by cardiologists and their research team.
The aim of this project is to provide dynamic remote monitoring for patients discharged after their first episode of acute heart failure, to improve and optimize guideline-directed medical therapy for acute heart failure patients. We believe the results of this study will help assess the feasibility of remote monitoring for acute heart failure patients in the future, to provide more out-of-hospital support and enabling earlier assessment and management before heart failure worsens.
All participation is voluntary, and you have the right to withdraw from this study at any time.
To assess the feasibility of remote monitoring for acute heart failure patients, this study may require users to provide some personal information such as gender and age through the application. The application will also collect data from remote monitoring devices, including:
By using this application, you indicate that you have agreed to comply with and be bound by this statement. The uploaded hemodynamic and biological data will be reviewed daily by the research team. Users have the right to terminate the provision of personal data and delete recorded data at any time. If needed, please contact the researchers.
This application is for research purposes only, to assess the feasibility of remote monitoring for acute heart failure patients. The medical and medication information provided by the application is for reference only. Research participants need to regularly contact and visit the research doctors and their team to confirm the treatment plan. The Chinese University of Hong Kong and its research team shall not be liable for any losses, claims, damages, costs, or expenses arising from the use of this application.
